An ERA workflow that actually closes the loop
Auto-posting 835s is the easy part. Here's the small set of rules that turns ERAs into a working A/R discipline.

Auto-posting ERAs feels like the finish line. It isn't. The finish line is a clean denial queue and an A/R aging report you trust.
Three rules for posted ERAs
- Every adjustment code routes to a named owner — not "billing."
- CO-45 contractual adjustments are auto-accepted only when they match the contracted rate within $1.
- Patient-responsibility transfers wait 24 hours before triggering a statement, so a same-day correction can catch it.
The weekly close
Friday afternoon: one person walks the denial queue from oldest to newest, and the A/R aging report is exported and diffed against last week's. Lines that age past 45 days get escalated, not re-worked silently.
